Each agency got a score from 0 to 5 on six criteria, multiplied by a published weight, for a total out of 100. Evidence came only from the agency’s own website, read on 4 October 2026. Where we found nothing, the agency scored low on that line. That is a gap in public evidence, not a judgement on its work.

What did Google AI Mode and Perplexity say?
Asked for the best healthcare marketing agencies in Saudi Arabia on 4 October 2026, Google AI Mode and Perplexity gave 4 answers that shared almost nothing. VOCTOS was named once, first, in Perplexity’s English answer on a VOCTOS company account. Neither AI Mode answer named VOCTOS, and AI Mode’s English and Arabic answers shared no agencies at all.
Our team ran each engine once in English and once in Arabic, between 14:00 and 14:35 Cairo time, from Chrome in Cairo. Google AI Mode was signed out and set to Saudi Arabia. Perplexity was signed in to a VOCTOS company account (Free plan), so personalisation was possible; read its VOCTOS result with that in mind. ChatGPT and Gemini were not run for this list.

“Unverified agency” means the engine named a firm we did not confirm, from its own website on 4 October 2026, as a healthcare marketing agency with a known headquarters, so we do not name it here. Only 2 of the 12 AI Mode names, Bytes Future and Social Clinic, made our list.
The VOCTOS result is weak, and we are publishing it anyway. Semrush gave voctos.com an AI Visibility score of 0 for Saudi Arabia when we checked on 29 and 30 September 2026. Getting a client like Delta Medical Labs cited by AI engines and getting our own agency brand cited are different jobs.

Each profile uses the same five lines: who it suits, where it is based, what its website offers, the evidence, and the score breakdown. Descriptions come only from each agency’s website as read on 4 October 2026. We did not contact any agency, and none paid to be listed.

What it offers: healthcare SEO built around named clinicians, a named medical reviewer with a visible review date, and medical schema, plus GEO for healthcare, technical SEO and Google Ads.
Evidence: for Delta Medical Labs, a Saudi medical lab, 1,186 keywords rank first in Saudi Arabia and 5,570 pages were cited by AI engines in July 2026 (3,600 AI Overviews, 1,800 AI Mode, 130 ChatGPT, 40 Gemini), on an engagement that started in March 2026. In Semrush AI Visibility for July 2026, Delta scored 18 against 14 for Al Borg Diagnostics and 14 for Wareed; only the Saudi Ministry of Health scored higher, at 22. مجموعة الحكيل الطبية reached 27,500 monthly organic visits in Saudi Arabia, 30 keywords at #1 and 128 pages cited by AI; that case study does not state a start date. What it offers: per its UAE website, healthcare SEO, Arabic SEO, paid media, bilingual content, right-to-left website builds and a dedicated healthcare AI and GEO visibility page covering Arabic and English answers.
Evidence: named Gulf healthcare projects, including an Arabic site for Royal Free London Private Patients covering 26 specialties, a website for CARE & Planning for Hospitals in Riyadh (launched 2023) and 2019 social campaigns for VPS Healthcare brands. Its GEO page labels these as marketing and web projects, not evidence of AI search results. It names the UAE regulators (DHA, DoH, MOHAP) but no Saudi regulator.

What it offers: per its website, SEO, PPC, social media, video, web development and personal branding for doctors and clinics, plus an AI search optimisation service aimed at Google AI Overviews, ChatGPT, Perplexity and Gemini. It says it built an Arabic content team to write patient content for the UAE.
Evidence: its case study library names 10 featured clients, including practices in Dubai and Bahrain, and says 80+ clinics work with it. One featured case, for a plastic surgeon in Vancouver, reports 1,000+ visits and 100+ leads in under two months, with no baseline; we found no Gulf case with result figures on that page. Its only compliance reference we found is “HIPAA-aware systems”, a US standard.

4. NEXA: 53/100

What it offers: its healthcare marketing page lists branding, websites, social, lead generation, HubSpot CRM setup and email campaigns. Its AI Lab sells a generative search optimisation service for AI Overviews, ChatGPT and Perplexity, which is not specific to healthcare.
Evidence: the healthcare page says its work with Invisalign and GMC Clinics won Best Use of Digital Marketing for Healthcare at the MENA Search Awards in 2018 and 2019. We found no healthcare result figures, no Arabic health content on the pages read, and no Saudi regulator reference; its GEO page mentions guardrails for regulated language in general terms.

5. Bytes Future: 51/100

What it offers: its medical and healthcare page describes SEO, social media management, digital advertising and website work for hospitals, medical centres and clinics. Its SEO page describes a dedicated Arabic SEO team.
Evidence: the healthcare page calls the agency “award winning” but names no healthcare client, award or result. Its SEO page shows a client logo wall that includes some health-sector names, without healthcare case studies. We found no GEO service and no reference to MOH or SFDA rules.

Evidence: its one healthcare work card names Al Nahdi and reports “+60% email open rate” on a newsletter campaign, with no baseline or timeframe. Several testimonials come from Saudi clients. We found no GEO service and no reference to Saudi health advertising rules.

7. ClinicScalers: 48/100

What it offers: per its Saudi healthcare page, medical SEO in Arabic and English, Google Ads, social media, local SEO, Arabic healthcare content it says native speakers write, bilingual website design, reputation management and CRM automation.
Evidence: the Saudi page names the Ministry of Health’s advertising guidelines and lists restrictions it follows on before-and-after images, treatment-outcome claims and patient testimonials. It does not mention the SFDA. We found no named client, case study or result figure on its homepage or Saudi page, and no GEO service.

8. Social Clinic: 48/100

Based in: Jeddah (Rovan Tower) and Riyadh (Al Yasmin). Established in 2011 as a social media consultancy, with over 120 staff working in Riyadh, Jeddah, Khobar and Madinah, per its site.
What it offers: per its website, advertising campaigns, content development, in-house production, media and technology, influencer and community management, crisis management and social listening.
Evidence: healthcare is listed among its client industries, but we found no healthcare case study on the pages we could read; its portfolio page did not load for us. SEO and AI search are not among the services listed on its homepage.

Which Saudi rules should a healthcare marketing agency know?
Two sets of rules matter most. Saudi law forbids health practitioners from advertising or promoting themselves except as its implementing regulation allows, and the SFDA requires prior approval before ads for medical devices, non-prescription medicines and other products it supervises. An agency that cannot explain both will put your licence at risk.
The practitioner rule sits in Article 10 of the Law of Practicing Healthcare Professions (Royal Decree M/59 of 4/11/1426H, published by the Ministry of Health). It also bars practitioners from showing titles or specialties they have not obtained, and its implementing rule 10-1 rules out commercial ads that are not based on scientific grounds. That touches doctor bios, ad copy and social posts.
The product rule comes from the SFDA’s statement of 27 July 2024: claims in an ad must be approved by the SFDA, the product must be registered or listed, and the ad must not mislead. A dermatology clinic promoting a device-based treatment can fall under both rules at once.

Why does healthcare content need medical review to rank?
Google says its systems give even more weight to content with strong experience, expertise, authoritativeness and trust (E-E-A-T) on topics that could significantly impact people’s health. A health page with no named author, no medical reviewer and no review date competes at a disadvantage, in classic results and in AI answers built from them.
The wording comes from Google’s guide to creating helpful, reliable, people-first content (page last updated 1 October 2026). Google also says E-E-A-T is not a specific ranking factor; the practical test is whether a reader can see who wrote and reviewed the page.
For Saudi providers the work happens in Arabic. Google brought AI Overviews to MENA and to Arabic on 20 مايو 2025, and Delta Medical Labs’ Semrush AI Visibility score of 18 in July 2026 sat behind only the Ministry of Health (22) among the Saudi health sites in that benchmark. Smaller providers can compete with large health portals inside AI answers, with Arabic pages that carry clinical review.

How do you choose a healthcare marketing agency in Saudi Arabia?
Choose on evidence you can check. Ask for one named healthcare result with a timeframe, a plain explanation of the MOH and SFDA rules that apply to your ads, proof of Arabic patient content written for Saudi readers, and how AI citations will be measured. Any agency that promises page-one rankings or guaranteed patient numbers should leave the shortlist.

- Which Saudi rules apply to our doctor pages and our ads, and who checks each ad before it runs?
- Who writes our Arabic patient content, and which licensed clinician reviews it before publication?
- How will you show author and reviewer names, review dates and medical schema on our pages?
- How do you measure whether ChatGPT, Gemini or Google AI Overviews cite our pages, and how often will you report it?
- How do you handle patient testimonials, before-and-after images and reviews within the rules?
- What will you not promise? (A good answer: no guaranteed rankings, traffic or patient volumes.)
- If we stop working together, which accounts, content and reports stay with us?

Our position: in Saudi healthcare marketing, a dated Arabic case study and a named medical reviewer are worth more than an office address or a large creative team. We say that knowing it partly favours us. Weigh published health results, compliance knowledge and Arabic clinical content first, then local presence.
The gap at the top of this list is larger than in our Saudi SEO ranking, and the reason is thin public evidence. Of the seven other agencies, none published a dated healthcare result with a baseline, and only ClinicScalers named a Saudi health regulator on the pages we read. Several are strong general agencies; their sites just do not show healthcare-specific proof.
The AI test points the same way: none of the six firms in AI Mode’s Arabic answer made our list, so check every AI-suggested name on its own site. A buyer who needs weekly meetings in Riyadh or Jeddah should move Bytes Future, NEXA or Social Clinic up, and that is a fair call.
Hiring for a clinic in the UAE or Egypt: what changes?
The checklist travels, but the regulator changes. Dubai clinics answer to the DHA, which re-circulated its social media medical advertising standard in August 2026; Abu Dhabi and federal approvals sit with the DoH and MOHAP. In Egypt, the Egyptian Drug Authority now publishes advertising violations online. Ask any agency which regulator it has worked with.

Four of the eight agencies above are headquartered in the UAE and a fifth has a Dubai-facing team, so a UAE buyer has more local choice than a Saudi one, but UAE compliance experience does not transfer automatically to Saudi rules.

Ask for healthcare SEO with Arabic pages for each treatment and branch, named doctors and medical reviewers on those pages, and Google Business Profile work for every location. Then ask for monthly reporting on organic clicks, bookings and AI citations, measured against a dated starting baseline, not on follower counts alone.
Does a healthcare marketing agency need an office in Saudi Arabia?
Not for search and content work, which runs on data, clinical review and technical access. A Saudi office helps with in-person shoots, hospital stakeholder meetings and local invoicing. It is which agency can show, with dates and named clients, that it has grown a Saudi health provider in Arabic within the advertising rules.
