Dr. Fahad Alebrahim’s clinic became the answer AI gives in Kuwait. Asked who the best gastroenterologist in Kuwait is, Gemini now recommends his clinic, and Google’s AI Overview names him among the country’s leading consultants. Behind that: 9 doctor searches at #1 on Google in Kuwait within the first month, and daily impressions in Google’s AI features up 3X in six weeks.

VOCTOS started the project in September 2026 and the work is ongoing. These are the results of the first two months, with the screenshots for each.

Why do AI assistants recommend one doctor over another?

AI assistants recommend the doctor whose facts they can read and verify. They repeat specific, consistent details: the specialty, the credentials, the clinic location. A doctor with excellent credentials but a website that crawlers struggle to read gives AI nothing solid to quote.

Dr. Fahad Alebrahim had the credentials: consultant in gastroenterology, hepatology, endoscopy and obesity treatment, American board certification in internal medicine, a gastroenterology fellowship at McMaster University in Canada, and head of Kuwait’s national programme for early detection of colon cancer. His clinic is at Al Hashel Medical Complex in Salmiya. The job was to make the website say all of that clearly to Google and to AI.

What was holding the website back?

In August 2026, none of the 48 Arabic searches VOCTOS tracks in Kuwait ranked #1 and only 2 were in the top 3. The audit explained why. The XML sitemap returned a 404 error, the homepage had structured data errors, a duplicate H1 ran across every page, more than 300 articles had no category, and a link audit found 594 link issues, from hreflang conflicts to internal links routed through redirects.

What did VOCTOS do in the first two months?

  • Made the site readable. Fixed the sitemap so it returns a 200 status, fixed the structured data errors, added a MedicalClinic schema to the homepage, removed the duplicate H1 and fixed all 594 link issues.
  • Gave each search one page. Arabic keyword research, a keyword gap analysis against competitors in Kuwait, a keyword map with one target URL per topic, and new meta titles and descriptions for the main pages.
  • Published what patients search for. 8 new articles built around search intent, a content audit of older articles, internal links between related topics, categories for more than 300 articles and a redesigned blog page.
  • Prepared for AI and measured leads. An llms.txt file for AI assistants and GA4 key events for phone calls and WhatsApp messages.

Which hidden problems were splitting the clinic’s authority?

Three problems spread the clinic’s authority across too many pages: articles that competed with each other, articles that competed with service pages, and two separate websites for one practice.

  • Overlapping, low-quality articles. A keyword cannibalisation audit across 305 articles and 26 service pages found thin posts and several articles chasing the same search. We cleaned them up so every topic now has a single page that owns it.
  • Blog posts outranking the service pages. Where an article and a service page targeted the same search, the service page now owns the booking intent and the article links to it.
  • Broken schema. Existing schema errors were fixed and Physician, MedicalClinic, LocalBusiness and FAQPage markup was added.
  • Two domains, one clinic. drfahadclinic.com and drfahadalebrahimclinic.com were competing for the same searches. Both are now in Search Console and are being consolidated into one primary site.
  • A shorter path to booking. A better services page experience and inbound WhatsApp and phone calls to action inside articles and pages, measured as GA4 key events.

The groundwork included a Screaming Frog crawl of all 333 URLs and a backlink gap analysis against three competitors, followed by 30 new backlinks and 10 reverse-engineered links in month one.

Which searches reached #1 in Kuwait?

Nine of the 48 tracked searches reached #1 in the September 2026 rank check, and the top 3 grew from 2 searches to 11. The biggest moves were on the searches a patient types when choosing a doctor.

What should other clinics take from this?

  • State the credentials plainly. AI repeats specific facts, like heading a national screening programme.
  • Fix the basics first. A broken sitemap or schema errors can hide an excellent doctor from both Google and AI.
  • One page for each search a patient makes. Best doctor, specialty and city searches each need a clear target page.
  • Track phone and WhatsApp contacts. That is how search turns into booked patients you can count.

Frequently asked questions

Can a clinic really be recommended by Gemini within two months?

Yes, when the doctor already has real credentials and the website states them clearly. On 24 October 2026, Gemini answered the question about the best gastroenterologist in Kuwait with Dr. Fahad Alebrahim’s clinic, less than two months after VOCTOS started in September 2026.

What did VOCTOS fix first?

The things that stopped Google reading the site: a sitemap returning a 404 error, structured data errors on the homepage, a site-wide duplicate H1 and 594 link issues. A MedicalClinic schema, a keyword map and new meta titles followed in the same month.

Is the project finished?

No. The work is ongoing. The next stage targets informational health topics such as acid reflux, IBS and fatty liver, plus backlinks from health sources in Kuwait and the Gulf.
